The Transitions of Care Clinical Coordinator will play a critical role in optimizing medication management for patients transitioning between care settings, reducing readmissions, and improving overall patient outcomes. This role requires expertise in 340B program compliance and optimization, as well as prior authorization (PA) processes to ensure medication accessibility and affordability for patients.
- Education Required : Doctor of Pharmacy Degree from an ACPE accredited school of pharmacy
- License(s) Required : Eligible for NYS licensure
- Experience / Skills Required : Minimum 2 years of pharmacy experience in transitions of care, hospital, retail or ambulatory settings. Board Certification in Pharmacotherapy and / or Ambulatory Care or PGY-1 and / or PGY-2 pharmacy residency in Ambulatory Care or Internal Medicine from an ASHP-accredited pharmacy residency program preferred.
