Key Responsibilities:
- Build working relationships with patients to understand their needs and connect them to appropriate resources both within the organization and in the community.
- Provide culturally and linguistically appropriate health education to promote physical and mental health and prevent disease.
- Support patients in learning to navigate the healthcare system independently.
- Collaborate with healthcare teams without providing clinical care or substituting for services offered by doctors, nurses, or social workers.
- Uphold the Mission and Core Values of St. Bernardine Medical Center and actively contribute to the hospital’s strategic plan and quality improvement activities.
This position requires strong interpersonal skills, a commitment to patient advocacy, and a proactive approach to supporting community health initiatives.
Qualifications
- High School Diploma, Associate Degree, preferably in a healthcare-related field preferred
- 1-2 years of experience as a community health worker or in a community setting
- Travel throughout San Bernardino County will be required.
- Experience in a healthcare setting working with patients, healthcare services, medical groups, and payors required.
- Bilingual Spanish speaker with ability to read and write Spanish preferred
- Demonstrate ability to navigate the internet and Windows based software applications. Able to provide culturally appropriate service. Ability to build effective relationships with patients, staff, community agencies, and community members. Knowledge of healthcare payors, medical groups, and services. Understanding of medical terminology. Good written and verbal communication skills
