Full Time Day shift, 80hrs/bi-weekly, rotating on-call, Benefit eligible for all benefits.
Job Objective
Assists in planning, organizing, implementing, evaluating, and directing the overall operation of St. Camillus Assisted Living and Memory Care Programs in accordance with current state federal, state, and local standards, guidelines and regulations, and the St. Camillus policies and procedures. Assists residents with adjustment to institutional living; assesses resident's psycho-social needs and creates/develops and carries out plan to meet the same. Acts as a liaison and referral person for resident and family problem solving. Is a compassionate advocate of the elderly.
Job Standards
- An associates degree or higher in in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology from an accredited college or university.
- Must have Standard Precautions and Medication administration and management prior to hire with successful completion of Fire Safety and First Aid and Choking within 90 days of employment to complete CBRF certification.
- Good verbal and written communication skills using tact, diplomacy and discretion.
- Working knowledge of community resources and ability to access these as appropriate.
- Able to work independently with little supervision.
- Ability to safely perform all essential job functions without causing harm to self or others with or without reasonable accommodation.
- Must have strong interpersonal skills, the ability to problem solve, and excellent conflict resolution abilities.
- Willing to assume on call responsibilities on a rotating basis.
- Minimum of one year of experience as a social work experience in a health care setting working directly with individuals.
- Computer skills required
- Current Social Worker licensure for the state of Wisconsin (CSW); preferred.
