Through special allocations, this position supports two programs with four service delivery ; the Kinship Caregiving Program and the Permanency Resource Center (PRC) Program, both programs are located in Albany's Capital Region and the Mid-Hudson Region of Upstate New York.

Position Overview
:


The Clinical Case Planner takes a leading role in the agency’s kinship caregiving and permanency resource center programs, ensuring that clinical supports are firmly in place for the identified families and individuals in need, and that the service delivery elements link to desired clinical outcomes for the programs.

This position works closely with the foster care director and other leadership of the programs to establish a set of clinical services, whether through regular group sessions or 1:1 support, that actively address the behavior health needs of the kinship caregiving community. The Clinical Case Planner reports to the agency’s Director of Foster Care and Kinship Services.


Position Requirements & Specifications:

  • Ability to work 40 hours weekly, with flexibility to accommodate client schedules as needed.
  • Minimum of a Master’s Degree in Social Work, Psychology, or Human Services. Candidates who are pursuing a Master’s Degree and have made substantial progress towards the advanced degree will be considered.
  • Preferred: minimum of 2 years of experience working with at-risk families and in community services.
  • Preferred: Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges, mental health diagnoses and trauma history.
  • Familiarity with family-centered systems of care and trauma-informed care.
  • Prior knowledge of mental health, juvenile justice, and prevention-focused programs.
  • Required: Demonstrated capacity to work with people with diverse backgrounds.
  • Local travel is required; A current Clean & Valid New York State Driver’s License (actively licensed at least one year) and reliable vehicle for transporting clients to community resources, meetings, etc. - with proof of 100/300 liability insurance coverage- is required.
    • Staff in driving positions must be able to meet the minimum requirements of our agency’s insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire
  • Experience with the (non-physical) de-escalation elements of Therapeutic Crisis Intervention (TCI) and trauma-responsive care, with the ability to respond effectively and therapeutically in crisis situations.
  • Strong interpersonal skills, including the ability to build strong working relationships with families, external organizations, and regulatory bodies.
  • Computer skills: Microsoft Office Suite; Google Business Workspace Suite; Secure Internet practices; Experience with AWARDS.
  • Excellent leadership, organization, planning and time management skills.
  • Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude.
  • A compassionate, warm and caring mindset; Capable of exhibiting empathy for those in crisis.
  • Capacity and openness to work with populations from culturally diverse and economically disadvantaged backgrounds, including but not limited to a broad range of people with various developmental and functional levels.
  • A professional record of integrity, proven reliability and dependability; Qualities of positivity, flexibility and adaptability.
  • Capable of exercising ethical, independent judgment and discretion while handling sensitive data with strict confidentiality.
  • Ability to pass background checks, including fingerprinting, required.

Essential Duties & Responsibilities include:

  • With the support of the agency’s Director of Foster Care and Kinship Services, provides clinical direction for the kinship caregiving and permanency resource center programs.
  • At the direction of the agency’s Director of Foster Care and Kinship Services, manages specific administrative aspects of the grant linked to data aggregation, performance reporting, etc.
  • In step with the program work plans, ensures that key clinical elements of the programs are delivered and the quality of services are at a high level.
  • Ensures that clinical services are in place for the identified families and individuals in need.
  • Refers families, from the kinship caregiving and permanency resource center programs, to more in-depth behavioral health support in the local community, as needed, to ensure long-term supports are in place for the families.
  • Leads the team in devising unique therapeutic activities that will engage the various members of the family in program services, bring the family together, and support their overall functioning.
  • Oversees implementation of the programs’ clinical elements, including strength-based assessments, service plan goal-setting linked to behavioral health issues, internal and external service referrals, etc.
  • Promotes the work of the case managers to guide them in implementing support groups, respite care, and other key program services, with an emphasis on a nurturing, therapeutic environment. Ensures that program staff members have the skillset (e.g., motivational interviewing skills) and training to sufficiently support the kinship caregiving families.
  • Proactively visits the caregiver homes, throughout the service catchment area, on a regular, prescribed basis, meeting face-to-face regularly with the programs’ caregivers.
  • Documents clinical services as required by St. Catherine’s and NYS-OCFS.
  • Completes other duties as assigned.
  • Other position-related duties as assigned by the supervisor.
