Join a dedicated team providing comprehensive, patient-centered care in a Medical Home Model. As an Advanced Practice Provider (PA or NP), you will diagnose, treat, and manage care for patients of all ages, collaborating with an integrated team of behavioral health, pharmacy, care managers, and specialists to ensure optimal outcomes.
Position Overview:
- Full-time, 40-hour workweek (minimum 36 patient contact hours)
- Participation in clinic call rotation (includes evenings, weekends, and holidays)
- Work closely with physicians, care teams, and community health educators
- Evidence-based Family Medicine practice with a focus on preventive care
- EHR documentation, coding, and billing compliance required
- Ongoing education and certification maintenance
Requirements:
- Master’s degree in Physician Assistant Studies or Nursing (Doctorate preferred)
- Unrestricted Oregon PA/NP license and national board certification (NCCPA, AANC, or ANNP)
- Federal DEA & BLS certification (ACLS required for sedation)
- Experience in Family Medicine, preferably 2 years in an outpatient setting
- Familiarity with Medical Home Care Model and EHR software (Epic preferred)
