We're looking for a dedicated Case Management Professional to join our team at St. Clair County, IL.
In this role, you'll work closely with applicants and participants to identify their strengths and challenges, develop personalized employment plans, and connect them with relevant resources.
You'll also be responsible for managing program budgets, authorizing payments for participant costs, and monitoring participant progress.
Main Duties
- Provide case management and career development services to assigned caseload
- Develop and implement individual employability plans (IEPs) for each applicant
- Enroll participants in relevant training programs and/or supportive services
- Manage program budgets and authorize payments for participant costs
- Monitor participant progress and make adjustments to their employment plans as needed
