Community Health Worker - Emergency Department
Position Overview:
Full CHW training and on the job training provided as needed.
Our Community Health Worker (CHW) will primarily be working with Emergency Room clients. The CHW will work to improve the health of patients among a broad area of health (healthcare, wellness, mental health, nutrition, disease management, etc.) and social services (housing, food, employment, etc.). This work will take the CHW into emergency rooms, out into the community, and to St. Clare Health Mission, working with this specific target populations. CHW's will work closely with medical providers, primary care teams, and other social service agencies to improve patient care and outcomes based in a local emergency department.
The Community Health Worker plays a key role in implementing care coordination and improving health outcomes for targeted populations. Through community care coordination this position will provide social, behavioral, and resource support to clients as they navigate the health care and social service systems primarily for high emergency department users.
This position will be primarily working with Emergency Department patients and in the community with targeted, yet diverse populations.
Requirements:
· Preference of 1 years health/social services experience and/or education
· Verifiable good driving record and reliable transportation
· Compassion, humility, empathy and a direct understanding of the conditions of poor and marginalized community members
Key Responsibilities:
Role Specific
· Convey the purposes and services of a program to the user population and the impact that program or service would have
· Help patients develop health management plans and goals
· Follow-up with health management/care plans with both patients and providers
· Coach patients in effective management of their chronic health conditions and self-care
· Assist patient in understanding care plans and instructions
· Provides support and educational activities for clients in order to promote health knowledge, self-sufficiency, self-care and to assist individuals in developing skills related to better health. Educational activities include but are not limited to: community resources, health education, social support, and daily living skills
· Assists in scheduling and accessing health and service appointments for client as needed i.e. financial, housing, transportation, primary care provider appointment, etc.
· Communicates with clients in-person, by telephone, electronically or by mail.
· Maintains knowledge of available community-based resources and programs to assist in meeting client needs.
· Motivates clients to be active and engaged in their health.
· Meets clients minimum monthly for appointments.
· Assists in ensuring services provide yield needed outcomes through education, communication, collaboration, and client advocacy, while preserving confidentiality.
· Acts as a liaison between the client and care team to support and provide resources for individuals
· Accompany clients to appointments as needed.
· Plays a consistent and active role in identifying project inefficiencies and finding collaborative solutions to the problems
Technology and Documentation
· Ability to learn and manage case management software and brief EPIC notes.
· Documents in a timely and accurate manner all encounters, referrals, and follow ups in an electronic database.
· Tracks client progress towards identified outcomes with community agencies and assist in obtaining services required for risk reduction.
· Follow through on assigned quality (QA) assurance activities.
· Maintain confidentiality of client-related information and maintain respectful treatment of clients.
· Accepts and performs other duties as assigned.
· Training, Experience & Other Requirements: Upon hire and completion of Community Health Worker certification be able to demonstrate an understanding of Community Health Worker (CHW) health care delivery system.
