LPN/CMA Allergy and Care Coordinator Nurse

The LPN/CMA Allergy Nurse serves as the primary support for patients referred by SCENT Physicians for allergy testing and ongoing immunotherapy. This position functions independently and is responsible for the daily operations and patient workflows of the Allergy Department. Other primary responsibilities include allergy testing, preparations of allergy serum, injections, and patient education..

The LPN Care Coordinator supports the clinic and provider care teams by delivering care coordination services by ensuring patients receive timely transitions of care by answering questions, educating, and guiding the patient and their families through the coordination of care process.

Hours: Monday through Friday, 8:00am-5:00pm. No nights or weekends.

Allergy Responsibilities

• Conduct initial patient interview following referral from ENT Physician to include educating patients regarding allergy testing and immunotherapy.
• Follow department policies and procedures pertaining to testing, treatments, and emergencies.
• Schedules and conducts percutaneous and intradermal allergy testing (MQT) as ordered by ENT Physicians; schedules and conducts ongoing immunotherapy.
• Prepare and maintain treatment and mixing boards; monitor for expiration dates.
• Prepare and mix subcutaneous immunotherapy (SCIT) and sublingual immunotherapy (SLIT) following USP 797 guidelines.
• Orders allergen supplies for the Allergy Department
• Maintain, clean and stock testing and treatment areas.
• Ensure emergency kits are in working order.
• Updates the patient chart with relevant medical data; documents in the EMR all pertinent patient information.
• Process payment and other billing documentation appropriately.

Care Coordinator Responsibilities

• Effectively communicate with staff, patients, and outside entities in a professional manner in-person, over the phone and electronically.
• Coordinate with RN Triage Nurses to assist in patient triage care, as necessary.
• Provide communication and function as a liaison between providers and patients, including working with multiple departments and external practices to ensure all patients receive the appropriate care.
• Follow up on all procedure and diagnostic orders to ensure appropriate patient follow up.
• Complete all paperwork associated with FMLA, Disability and Workers’ Compensation and forward the paperwork to the appropriate source on a timely basis.

Required Education and Experience

This position requires successful completion of an accredited school of practical nursing and current and unrestricted licensure as a Licensed Practical Nurse in the state of Minnesota; or current certification as a Certified Medical Assistant. Current BLS for Healthcare Providers license is required.

Candidates with at least two years of prior experience in medical practice preferred.

Physical Demands

• Will involve moderate amounts of walking and standing, and repetitive actions in using the computer keyboard and continuous viewing of information on the computer monitor.
• At times, may need to assist with patient’s mobility difficulties, therefore is required to be able to lift, squat, bend, and reach.

Additional Eligibility Requirements

Must be able to pass a criminal background check and complete Tuberculosis screening and show proof of Hepatitis B vaccination series.
