CentraCare is seeking an experienced and compassionate Registered Nurse to join our team as a Transitions Coach. In this role, you will work closely with patients, empowering them to better engage with the healthcare system, set and achieve personal goals and ensure they understand their post-discharge care plan. As a Transitions Coach, you will play a key role in helping patients navigate their healthcare journey, ensuring they feel supported, informed and confident in their recovery process.
Schedule:
- Full-time | 72 hours every two weeks | Remote position
- Monday - Friday | 8:00 am - 4:30 pm
- No weekends or on-call required!
Pay and Benefits:
- Starting pay begins at $83,886.40 per year; exact wage determined by years of related experience
- Salary range: $83,886.40 - $125,840.00 per year
- Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

Qualifications:
- Graduate from an accredited school of nursing with a bachelor's degree or higher required
- Current Minnesota Registered Nurse license
- Minimum of 5 years of clinical nursing experience with diverse patient populations
- Current American Heart Association (AHA) BLS Healthcare Provider card
