St Croix Hospice is hiring an RN Case Manager for our Rockford branch. The territory will be primarily rural areas west of Rockford.

This role requires a rotation of weekend on call.

What will you do?
The Registered Nurse (RN) Case Manager plans, organizes and directs hospice care with emphasis on community health education/experience. You will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
- You will complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
- You will complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.
- Travel throughout the day and sometimes after-hours
- Case Managers also work an on-call schedule on a rotating basis.

Qualifications
Required
- RN licensure for the state(s) you work in
- Clean driving record, and valid driver’s license and auto insurance
- 1 years of recent acute nursing experience and 1 years or recent hospice/home care experience
The starting salary range for this role is $78,000-$97,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
