Position Title
CFSS Consultation Service Coordinator
Compensation (Pay/Benefits) Our employees are our most valuable asset; therefore, we offer competitive pay and benefits. The pay range for these positions starts at $49,878.40-$58,177.60 a year for full time, depending on qualifications (i.e., licensure and experience). This salary reflects a 40-hour workweek, annualized. Any agreed-upon reduction in weekly hours will be reflected in salary commensurately. Please inquire with interest. This position is benefits eligible, including health and dental insurance, health savings account with an employer contribution, flexible spending accounts, and retirement account with matching contribution. Additionally, we offer generous PTO, paid holiday schedule, childcare discount, and eligibility for loan forgiveness!
Schedule
Full time, Monday-Friday 8am to 4:30, this position is a hybrid work model that is mostly remote but will require some in person meetings and office work.
Location: Remote - work from home.
Program Summary
Community First Services and Support (CFSS) is a service that replaces PCA (Personal care attendant) and CSG (Consumer Support Grant) services. In this new service people will have a choice in self-directing their own or their loved one's care through staffing and/or goods and services.

Education
A bachelor's degree or higher in a discipline associated with at least one of the following fields of study: Human behavior, Human skill development, Humans and their cultural behavior, Human condition, or any other study of services related to basic human care needs. Will except equivalent experience in leu of a degree.
Experience
Two years of full-time work experience providing direct services to people with disabilities or people over 65; or experience coordinating or directing services for people with disabilities or people over 65 including self-directed services or experience coordinating their own services is required. One year's experience in case management or coordination of services for individuals/families with developmental disabilities is preferred; preference includes experience with PCA, CSG, DD Waiver, developmental disabilities services.

- Provides overall guidance for clients and families who are new referrals to St. David's Center's Consultation Services which results in individuals and families feeling informed and welcomed.
- Ensure timely client enrollment in Consultation Service program in accordance with program requirements through the processing of referrals.
- Provide education to help people make informed decisions about how to meet their needs using CFSS.
- Help the person write their CFSS service delivery plan, to the extent the person desires.
- Provide ongoing support as needed.
- Document services provided utilizing EMR.
