Key Responsibilities
- Conduct thorough assessments of patients' physical, psychosocial and environmental needs.
- Develop and implement personalized care plans to ensure optimal patient outcomes.
- Serve as a clinical resource for home health aides and LPNs/LVNs, providing guidance and support as needed.
- Participate in on-call rotations and emergency call according to agency policy.
Requirements
- Graduate of an accredited school of professional nursing.
- Minimum one year of home health, public nursing or acute hospital nursing experience.
- Familiarity with ICD-10 coding and PPS.
- Valid driver's license and reliable transportation.
- Current BCLS Certification.
