Job Summary and Qualifications
The Physical Therapist provides high-quality, patient-centered care by managing the complete process of therapeutic care for assigned patients. This includes completing patient assessments, participating in developing individualized care plans, and providing or directing patient physical therapy.
Responsibilities include:
- Providing evaluations and treatment interventions within the scope of established practice.
- Assessing and evaluating therapeutic/rehabilitative/functional status and participating in developing the total plan of care.
- Providing purposeful and skilled therapeutic care at patients' homes using procedures and techniques consistent with the physician's plan of care.
- Instructing patients/families/caregivers in therapies, exercises, therapeutic appliances, and/or prosthetic devices for use outside of therapy sessions.
- Prioritizing caseloads, determining daily staffing needs, and coordinating assignments/handoffs.
Qualifications
- Bachelor of Science in Physical Therapy from an accredited program.
- Certifications, Licenses, and other Special Requirements.
- Completion of the National Certification Examination for Registered Physical Therapist.
- License to practice physical therapy within the state.
- Ability to work varying hours, including weekends and holidays.
- Must be a licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working order.
