Certification Requirements
The following certifications are required for this position:
- BLS (Basic Life Support)
- Nursys - Nursing license and certification, verified within 30 days of start
- FMOLHS Skills Checklist - Unit specific to the contractor's scope of practice, completed within one year prior to start date and renewed annually
- FMOLHS Competency Exam - Copies of exams needed, completed within year of start with passing score of 80% or higher
- FMOLHS Core Competencies - HIPAA, Patient Bill of Rights, Safety Orientation, Blood borne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually
- FMOLHS MS Fingerprinting - State requirement for Mississippi
- MMR - Proof of 2 immunizations, positive titer or Declination accepted
- Fit Test - Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870 . Completed within 1 year prior to start
- Hepatitis B - Proof of 3 immunizations, positive titer or Declination accepted
- Influenza Vaccine - Proof of vaccine required from October - March
- TB Test - Proof of negative Blood test or Skin test
- Health Statement - Statement signed and dated within 1 year of start stating traveler is able to work without restrictions
- Varicella - Proof of 2 immunizations, positive titer or Declination accepted
- Tdap - Proof of vaccination within past 10 years or Declination accepted
- Annual TB Questionnaire - Required within 6 months of start
- FMOLHS Criminal Background Check Results - Background checks must be within 30 days prior to start
- FMOLHS Driving Record - Can be included in Background Check
- FMOLHS Drug Screen - Negative 10 Panel within 30 days prior to start
