- FMOLHS MS Fingerprinting - State requirement for Mississippi.
- MMR - Proof of 2 immunizations, positive titer or Declination accepted.
- Fit Test - Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870 . Must be completed within 1 year prior to start.
- Hepatitis B - Proof of 3 immunizations, positive titer or Declination accepted.
- Influenza Vaccine - Proof of vaccine required from October - March.
- TB Test - Proof of negative Blood test or Skin test.
- Health Statement - Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
- Varicella - Proof of 2 immunizations, positive titer or Declination accepted.
- Tdap - Proof of vaccination within past 10 years or Declination accepted.
- Annual TB Questionnaire - See document library. Required within 6 months of start.
- FMOLHS Criminal Background Check Results - Background checks must be within 30 days prior to start.
- FMOLHS Driving Record - Can be included in Background Check.
- FMOLHS Drug Screen - Negative 10 Panel within 30 days prior to start.
