Job Summary:
St. Elizabeth Home Care is seeking a highly skilled and compassionate Physical Therapist to join our team. As a Physical Therapist with our organization, you will have the opportunity to work with patients in their own homes, providing personalized care and rehabilitation services that promote optimal health outcomes.
Responsibilities:
- Clinical Excellence: Develop, implement, and supervise home care plans for physical rehabilitation of patients under the direction of attending physicians and clinical branch leadership.
- Patient Assessment: Perform initial evaluations and establish care plans, including environmental modifications, based on patient assessments. Provide physical therapy treatments according to established plans of treatment.
- Communication: Report changes in patient condition to appropriate personnel and attending physicians. Educate patients and caregivers on relevant therapies and interventions.
- Collaboration: Participate in coordination of services with other disciplines and community resources as needed. Supervise physical therapy assistants (PTAs) as prescribed by state and federal regulations.
Qualifications:
- Educational Requirements: Bachelor's degree from an accredited school approved by the American Physical Therapy Association.
- Licensure: Currently licensed to practice in the state serviced.
- Professional Development: Maintain current CPR certification and stay up-to-date with industry developments.
- Skillset: Strong communication and time management skills, passion for learning and teaching, and proficiency in using technology.
