General Summary:
The Licensed Practical Nurse provides comprehensive nursing care to seriously ill patients or less comprehensive care to a greater number of patients. Helps coordinate patient care within the healthcare setting under the supervision of the Charge Nurse or Nursing Supervisor. Supervises Certified Nursing Assistants/Nursing Assistants assigned to her/his team.
General Duties and Responsibilities:
- Maintains high standard of nursing care for her/his patients;
- Observes and gathers information for all patients under her/his care, reporting unexpected findings to Charge Nurse or Nursing Supervisor;
- Accurately and objectively documents all observations, information gathered, and care given;
- Participates in doing all necessary charting;
- Receives daily pharmacy delivery and signs for narcotics as needed;
- Receives physician orders from Nursing Supervisor and transcribes to Medex;
- Performs patients’ treatments as ordered by physicians; distributes medications;
- Counts controlled medications at the beginning and end of her/his shift;
- Completes Medication Sheets and changes same when indicated;
- Reviews monthly medicine sheets;
- Directs patient care as indicated in Care Plan;
- Supervises and assists Certified Nursing Assistants/Nursing Assistants assigned to her/his team in doing patient care as needed;
- Directs the staff in obtaining monthly vital signs and weight;
- Responds to call bells and meets the requests of the patients to the best of her/his ability;
- Attends report and prioritizes sisters’ medical needs, checking them promptly and following through with pertinent and timely care, and notifies Registered Nurse if any change in condition;
- Counsels staff when necessary, assuring that verbal or written warnings are given properly;
- Assists in orientation and training of new Licensed Practical Nurses and Certified Nursing Assistants/Nursing Assistants;

- Serves as Charge Nurse when requested;
- Maintains a cooperative, pleasant attitude toward all: patients, coworkers, sisters, and visitors;
- Adheres to Departmental Policies and Procedures;
- Knows and adheres to established Personnel Policies, Fire and Disaster Plans;
- Maintains confidentiality;
- Attends In-Service Programs and Staff Meeting;
- May assist in evaluation of Nursing Assistants;
- Carries out other appropriate duties as requested by the Nursing Supervisor.
Qualifications:
Education:
- Graduate of an approved School of Practical Nursing and licensed to practice in the State of New York;
- Preferred: courses or In-Service in Geriatric Care.
Experience:
- Six months in a hospital or nursing home;
- Preferred: includes experience in Geriatrics.
Other:
- Evidence of good health;
- Adequate vision; good hearing;
- No previous serious back injury;
- Emotional stability;
- Must be able to lift and transfer patients.
