Supportive Housing Case Manager
Objective: The Supportive Housing Case Manager is responsible for providing follow up case management services tothe Emergency Shelter and Casa Familia Programs with the goal of assisting clients in maintaining their housing, and to provide support as needed to residents and clients at all St. Elizabeths Programs as needed
Responsibilities and Duties
St. Elizabeth Shelter Supportive Housing Case Manager
- Provides follow-up case management to Emergency Shelter and Casa Familia clients after they obtain housing.This includes an initial phone call to all clients within the first week of obtaining housing and a follow-up by month's end. For those doing well further contact will be limited. For those assessed as high-risk, theSupportive Housing Case Manager will conduct at least monthly home visit or more if needed.
- During house visits, the Supportive Housing Case Manager is to screen the clients for obvious and potential problems - mental and physical health, alcohol or drug usage, medication management, paying rent and budgeting, diet, isolation, condition of housing, etc. and to intervene as appropriate to help maintain their recovery, self-sufficiency and well-being. The goal is to provide the same level of case management support that clients currently receive while staying at our emergency shelters.
- The Supportive Housing case manager is responsible for client's progress notes, case management notes on file maker.
- Assesses clients for appropriateness for referral to outside agencies.
- Meets, as needed, with staff of other agencies to provide wrap-around services for clients.
- Attends periodic staffing meetings with the case management team to review specific clients, overall protocols, issues and areas needing improvement.
- Provide support at Santa Fe Suites 25 hours a week or more, follow up with notices from property management or tenants identified to need extra support to maintain housing from service provider staff or management, and as needed when staffing is limited especially with community events.
Requirements
Credentials/Qualifications
BS Degree in social work, counseling, psychology, or related field preferred, or equivalent work experience
New Mexico licensure such as LMSW, LADAC, LPC or equivalent.
Experience and Skills
Spanish speaking preferred
Experience working with homeless individuals and/or families preferred
Willing and able to work with diverse populations
Effective communication skills
Computer proficient
Knowledge of the unique cultural heritage of northern New Mexico
Other
- Valid driver's license and proof of insurance
- Willing and able to use personal vehicle for local travel on agency business
- Willing and able to work irregular hours and have a flexible work schedule
Job Type: Full-time
Pay: $23.63 per hour
