St. Francis is NOW HIRING for a Case Management Manager to help lead our Case Management Team!
This is a Monday through Friday position with on-call to assist via phone as needed.
The Manager of Case Management serves as a resource for the case management team and other departments. Responsible for providing direct supervision of the staff of the case management department
Additionally you will:
- Provides support and oversight to the case management staff, providing coaching and mentoring as needed
- Collaborates with the Director of Case Management to ensure staff completes annual competencies and education as assigned
- Effective and appropriate communication with case management staff, providers, healthcare team, patient, and family
- Assists the Director of Case Management with collection of data for the UM Committee and other reports
- Collects data and other metrics as requested by the Director of Case Management. Monitors productivity and regulatory compliance
- Provides appropriate guidance to staff and others
- Establishes and maintains effective relationships with department heads, nursing managers and staff, administrative staff and administrators
- Displays role modeling behavior
- Establishes clear expectations around accountability and ownership
- Sets clear expectations in alignment with organizational goals
- Contributes to workload distribution
- Assists with onboarding new hires
- Participates in Interdisciplinary Rounds
- Displays effective communication skills
- Supports the Director of Case Management and assists with other job duties as requested
- Demonstrates knowledge of HIPAA guidelines and utilizes them in all aspects of communication

Qualifications
Qualification
- BSN or MSW required
- Licensed as a RN or LCSW in the state of Georgia, required
- 3 years of acute care hospital case management experience or 5 years in a related healthcare setting
- 2 years of utilization review experience, preferred
- Previous leadership experience, required
