Discharge planner provides information to Social Work and Case Management Department staff regarding changes to providing direct and indirect client care; providing psychotherapeutic services to clients and families; serves as the primary liaison between the hospital and outside agencies for discharge planning. Ensure patient assignments are maintained and documentation protocols are followed. Responsible for ensuring all opportunities for expense reimbursement through commercial and government payors is fulfilled.
Qualifications
Minimum Education
X Graduate of a Practical Nursing Program X Required
*** 5 years of experience in a home health, nursing home, hospice or acute setting role may be considered for substitution of the LPN or BSW requirement.
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Basic computer skills and ability to learn and utilize Cerner computer software
Ability to work as a team member.
X LPN or BSW or 5 years experience in a related field. X Required
X LPN or BSW with 2 years of acute care work experience. X Preferred
