What are the responsibilities and job description for the Triage RN position at St. Francis Reflections?
Under the general direction of the Clinical Manager, the Telephone Triage Registered Nurse is responsible for answering and documenting telephone calls from patients/caregivers/facilities, assessing the severity of the patient’s symptoms, triaging call appropriately via phone, coordinating further interventions through SFR visit staff, and making follow up calls to assess changes of status due to interventions. May complete visits as needed.
EDUCATION AND/OR TRAINING REQUIRED:
Requires an Associate degree in nursing or a diploma certificate from an approved nursing program. Bachelor’s degree preferred.
CERTIFICATIONS/LICENSES REQUIRED:
Requires a valid current Florida Registered Nurse License and a valid Florida Driver’s License.
EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:
One to two years experience in home health, oncology, acute care or public health. Ability to read, write and speak English. Bi-lingual preferred. Ability to manage time and triage phone calls received from patients, families and physicians. Excellent communication and interpersonal skills.
PHYSICAL REQUIREMENTS:
Ability to lift, push, pull twenty five (25) pounds without difficulty. Good visual and audio acuity. Ability to work effectively under varying degrees of stress.
PRIMARY RESPONSIBILITIES:
- Possess, maintain, and apply primary nursing knowledge of hospice care while performing all aspects of assigned tasks. Must use critical thinking skills.
- Understand the dynamics of human behavior and ability to communicate with patients and families in a crisis.
- Conduct thorough and sound nursing assessments of callers presenting symptoms.
- Develop, coordinate, implement, and evaluate a plan of care for each caller presenting symptoms.
- Follow organization’s nursing protocols and procedures consistently.
- Answer all calls in a timely manner per written guidelines.
- Provides emotional support to patient/family and other caregivers.
- Interact with all callers and staff in a polite and courteous manner.
- Maintain confidentially of all caller and personnel issues, as appropriate.
- Documents all calls according to department policies and procedures.
- Complete patient visits as requested.
