St. Gabriel's Hospice and Palliative Care is seeking a compassionate and dedicated PRN Social Worker to join our team in the north part of Houston, Texas. Key Responsibilities:
- Conduct comprehensive psychosocial assessments for hospice patients
- Develop and implement individualized care plans to address the emotional, social, and practical needs of patients and their families
- Provide counseling and support to patients and families facing terminal illness, grief, and loss
- Collaborate with the interdisciplinary team to ensure holistic and patient-centered care
- Facilitate access to community resources and services as needed
- Maintain accurate and timely documentation of all patient interactions and interventions
- Participate in team meetings and case conferences to discuss patient care and progress
- Master's degree in Social Work (MSW) from an accredited institution, preferred
- Current Social Work license in the state of Texas
- Minimum of 2 years of experience in hospice, palliative care, or a related field, preferred
- Strong understanding of the psychosocial aspects of end-of-life care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of an interdisciplinary team
- Compassionate and empathetic approach to patient care
- Flexibility to work on an as-needed basis (PRN)
