DSW Supervisor - MUST have supervisor experience
Home and Community Based Wavier Services, Inc. experience is a MUST.
Supervises direct care staff in training programs
Checks all charts and schedules and is responsible for making sure that direct care staff are charting information correctly.
Responsible for scheduled visit verification, auditing notes, skills checking, making sure all employees records are accurate and up to date according to state guidelines.
Attends all scheduled meetings made by the support coordinator
Performs consumer specific training for direct support staff and ensures documentation is correct and placed in DSP’s personnel file.
Keeps all consumer’s homes staffed with back up workers in place
Notifies family of changes in services, revisions, CPOC beginning and ending dates and ensure all information is accurate.
Travels as required to consumer’s homes and attending meetings as necessary
Updates plan of care prior to their expiration
General clerical duties, office duties, such as filing, answering phones, etc.
Each week, supervisor will ensure that prior authorizations, hours worked and progress notes coincide with one another.
Supervisors will be on call as scheduled
Supervisors are to remain professional at all times, no use of profanity or any type of derogatory language is to be used.
Rotate weekend on call
