Job Title: Licensed Practical Nurse (LPN) II

Department: Clinical Areas

Reports To: Department Manager

FLSA Classification: Non-Exempt

Job Summary: The Proficient Licensed Practical Nurse, under the direction of the Nurse Manager or designee and the co-assigned Registered Nurse, is accountable for the provision of direct care to assigned patients. The Proficient Licensed Practical Nurse contributes to unit activities and supports the enhancement of the care delivery system at the unit level. The Proficient Licensed Practical Nurse works as a team member and assists other staff members with implementing patient care. The LPN is responsible for the delivery of care which is performed in a professional, caring, and courteous manner. Responsible for making safety-driven decisions while providing patient care. Participates in department-wide team initiatives.

Delivery of High-Quality Clinical Outcomes

  • Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD
  • Documents collected data, provision of care, and medication administration according to unit and service standards; Participates in conjunction with the assigned RN in patient education as outlined in the plan of care.
  • Safely and effectively administers and documents medications according to hospital policy utilizing appropriate methods.
  • Assists in the identification of patient and family needs; Participates in the plan of care, implements appropriate nursing interventions and contributes to patient/family education through a collection of data, interdisciplinary team collaboration, documentation, and excellent communication
  • Identifies basic age-specific, cultural, and spiritual needs of patients and families; Clearly communicates these needs to appropriate members of the care team; Recognizes obvious changes in patient status-seeking appropriate resources to address concerns
  • Consistently works to promote safe and effective patient care
  • Applies healthcare practices as deemed competent by nursing educators and under the direction of a provider.
  • Interprets, supports, and implements policies and procedures.
  • Acts as a team player and works collaboratively within the team and organization.
  • Assists with developing priorities, goals, and action plans.
  • Responds to requests from Department Manager.
  • Collaborates with interdisciplinary teams to assist with the evaluation of programs and services.
  • Participates in department and/or system-wide team dynamic initiatives.
  • Accurately records observations.
  • Liaison for provider and patient/family
  • Communication and decision-making skills

Position Qualifications

Education & Experience

  • Graduate of an approved School of Practical Nursing with a permit or license (LPN) to practice in New York State.

Licenses and/or Certifications

  • BLS Required

Physical Demands

Ability to move or transfer patients with an individual effort equal to 50 lbs. Should have a full range of body motion to successfully perform all essential functions of the job, meaning you’ll have to stand, walk, bend, kneel, crouch, and reach frequently during a 12-hour shift.

Language Ability

Ability to read, write and interpret documents in English such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read, analyze and interpret common scientific/trade/technical journals. Ability to respond to common inquiries or complaints from customers, or regulatory/agencies. Ability to effectively present information to employees, top management, public groups, and/or board of directors

Math Ability

Ability to add, subtract, multiply and divide into all units of measure, using whole numbers, common fractions, and decimals. Ability to calculate figures and amounts such as proportions, percentages, rates, and ratios.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardizations exist. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Working Conditions

Potential exposure to bio-hazardous materials, toxic chemicals, and communicable disease. Handles emergency or crisis situations, may perform emergency care. Occasionally subjected to irregular hours, contact with patients under a wide variety of circumstances.
