Job Duties

The HCN Program Manager is responsible for the direction and supervision of LPNs and nursing assistants in our Highland Court Nursing Memory Care Area, ensuring the provision of quality care and health services, and compliance with professional nursing standards of practice and state and federal regulations. The HCN Program Manager will take the lead in working with current programs, including care planning will also work in conjunction with the Life Enrichment Manager to maintain ongoing activity programs to meet the special needs of the residents. Ensures resident safety, security and rights at all times.

Resident Care and Services

  • Coordinate resident move-ins, transfers, and move-outs with admit team
  • Initiate assessment, care plan, intervention/treatment and outcome measurement within regulated timeframe
  • Oversees LPNs and CNAs in the provision of effective, quality care and services to the geriatric population with Alzheimer’s/dementia-related conditions
  • Consult with attending physicians to provide quality resident care
  • Monitor the communication by nursing staff of resident status to ensure quality resident care
  • Monitor the requisition and facilitation of diagnostic and therapeutic services by the nursing staff, as ordered by the physician, and in accordance with established procedures and protocols
  • Monitor the reporting of all discrepancies noted in physician orders, charting or medication treatment administration
  • Review resident chart for accuracy and completion; monitor the completion and filing of forms/charts from move-in to move-out in accordance with state and federal regulations
  • Monitor the completion of nursing notes that reflect residents' status, care provided, and response to care
  • Provides wound care, as ordered, to residents of HCN in collaboration with the RN Wound Care Nurse; designates others (LPNs) to provide wound care as needed.
  • Communicate and collaborate with team members to promote quality resident care and services
  • Monitor the delivery of professional nursing services such as assessment, treatment and outcome evaluation, medication, administration parenteral and enteral therapy, airway maintenance, wound care, care of dying resident; ensure compliance with professional nursing standards of practice
  • Develop positive relationships with family and significant others and include them in resident assessment, care planning and treatment when appropriate
  • Work in conjunction with the Life Enrichment Manager to maintain on-going activity programs to meet the special needs of the residents
  • Ensure the accurate documentation of all resident accidents/incidents
  • Ensure resident safety, security and rights at all times

Leadership Functions

  • Ensure that the overall appearance and environment of the HCN area is inviting, neat and sanitary; promptly notify the appropriate departments of environmental or maintenance concerns
  • Provide effective supervision, motivation, and leadership to staff
  • Provide effective training to staff in areas of oversight and delegate duties as needed
  • Ensure accountability of staff and apply effective coaching, counseling, and disciplinary actions as needed
  • Conduct timely performance reviews for staff
  • Participate in the interview and selection process of new staff and ensure adequate staffing
  • Ensure that employee related paperwork is submitted timely to HR
  • Facilitate, attend, and participate in various departmental and management meetings as required
  • Participate in corporate outreach activities, resident events, and staff events
  • Participate in professional development and continuing education opportunities
  • Monitor departmental supplies and inventory and ensure proper storage of equipment and supplies

Education/Training/Experience

  • Graduation from an accredited school of Licensed Practical Nursing
  • Possess an unencumbered license to practice Licensed Practical Nursing in Louisiana
  • Minimum of 3 years nursing experience, preferably in dementia related care
  • Current Health Care Provider CPR certification
  • Nursing supervisory experience preferred
  • Proficient computer skills; ability to utilize resident care software programs and Microsoft office applications

Physical Requirements: Ability to stand and walk for extended periods and the ability to utilize proper body mechanics while sitting at a desk for an extended period.
