Job Description:
As a member of our interdisciplinary team, you will be responsible for providing high-quality patient care in our Skilled Unit (SNF) and Long Term Care Unit. You will have the opportunity to collaborate with physicians, NPs, families, and other healthcare professionals to develop and implement comprehensive care plans.
Responsibilities:
- Deliver direct patient care, including medication administration and vital sign monitoring.
- Participate in multidisciplinary rounds and conferences to discuss resident care plans.
- Provide education and support to residents and their families.
