St. James Rehabilitation & Healthcare Center is seeking a talented Social Worker/Discharge Planner to join our team
Social Worker/Discharge Planner Job Description:
The position requires attending daily morning meetings. weekly UR meetings, leading, planning and completing all discharges. Gathering discharge information from new skilled patients within 48 hours of admission. Planning and attending discharge care plan meetings, addressing discharge concerns, documenting discharge notes in ECS, completing discharge audit forms, assist with annual post form audit.
Discharge Planner QUALIFICATIONS/REQUIREMENTS:
- Bachelor's degree in Social Work or related field (Psychology, Nursing or Care Management)
- Experience in Social Service office preferred, but not required

Salary: Up to $80,000 a year
