Description:
Responsible for assisting homeless individuals in obtaining and maintaining permanent
housing and providing the ongoing support needed to maintain a stable living
environment utilizing the principles of Housing First, Trauma-Informed and Person-
Centered Care, Motivational Interviewing, Strengths-Based Case Management, and
Harm Reduction. Contribute to additional single-site permanent supportive housing
start-up tasks as needed.

Responsibilities:
Work responsibilities will include but are not limited to:
- Identify client needs related to housing via engagements, interviews, observations,
and assessments
- Conduct ongoing outreach to individuals experiencing homelessness
- Develop individual client service plans and document client/worker activity
- Conduct at least monthly home visits to clients in permanent housing
- Conduct life skills group meetings and coordinate annual events and celebrations for
clients in permanent housing
- Make referrals and assist clients in connecting to community resources
- Accompany clients and/or provide transportation to service providers
- Conduct apartment searches, maintain landlord relationships, and act as liaison between landlords and clients in obtaining and maintaining housing
- Monitor the delivery and outcome of services provided to clients by medical facilities, social service agencies, and community resources
- Provide crisis intervention as necessary
- Complete documentation required by appropriate funding sources
- Participate in staff and community meetings
- Act as a member of the St. John Center frontline team and participate in daily functions and organizational responsibilities of St. John Center
Physical Requirements:
- Ability to travel throughout Metro Louisville to aid in apartment searches and perform home visits which may require ascending/descending a full flight of steps, moving around in tight spaces, and other physical movements
- Standing or sitting for long periods, bending, light lifting, walking, climbing stairs
Minimum Qualifications:
- Bachelor’s Degree in a behavioral science
- Basic computer literacy including word processing and Google suite
- Effective communication skills
- Must have a personal automobile, valid driver’s license, liability insurance, and willingness to transport co-workers and/or clients
Preferred Qualifications:
- Be an approved Targeted Case Manager with the state of Kentucky
- 5 years of experience in working with the homeless population
- Ability and willingness to safely and effectively de-escalate situations, including conducting physical interventions when necessary to maintain safety as directed by the agency
Job Type: Full-time
Pay: From $41,000.00 per year
