- Complete patient intakes by phone and electronically.
- Upload documents and maintain accurate patient files.
- Set up telehealth calls between patients and providers.
- Assist patients and family members with their needs by phone and virtually.
- Monitor treatment orders for timely delivery.
- Keep providers equipped so that they can serve patients in retirement communities.
- Monitor patient cases so that no patient gets forgotten. Qualifications :
- Must be highly influential on our amazing senior patients to help them get the CARE they need.
- Must be comfortable and capable of navigating and maintaining patient records in practice management software.
- Proficient in office software and email.
- Analytical skills to maintain accuracy.
- Personal relationship skills to serve patients and families.
