Full-time - shift varies depending on staffing needs - on-call 7 days / month

SUMMARY : Responsible for creating a nurturing home for residents and provides care and services to meet their needs within an environment of hope, dignity, and love. The CNA Team Lead works cooperatively and respectfully with residents, coworkers, families, medical staff, visitors, and volunteers to create this positive environment.

ESSENTIAL JOB FUNCTION : Staff Coordination

  • Is the liaison between the team and families, volunteers, community members, and all other members of the staff.
  • Is responsible for calling and conducting Team Meetings.
  • Is responsible for communicating team decisions to staff.
  • Is proficient in OnShift scheduling program and understands scheduling requirements.
  • Understands staffing requirements and is able to acquire help to cover openings or call offs and to flex schedules to assist with coverage if necessary.
  • Will attend OnShift trainings as offered to maximize efficiency.
  • Attends leadership training as assigned by supervisor and any suggested AE training or SJU manager / supervisor training.

ESSENTIAL JOB FUNCTION : Staff Support

  • Communicates job expectations to team.
  • Works as a CNA a minimum of three days a week to understand resident care needs and to work together with coworkers.
  • Ensures that each team has appropriate staff levels. Assists staff with finding coverage when necessary.
  • Maintains quality service by complying with State and Federal Regulations governing employment law and Montana DPHHS Assisted Living Regulations and Quality Assurance Regulations.
  • Assists with monitoring, and appraisal of job performance under the supervision of Assistant Director.
  • Coaches employees under the supervision of Assistant Director.
  • Ensures all established policies are appropriately adhered to.
  • Assists in the hiring process of new team members.
  • Provides mentorship to employees and makes themselves available to train with new employees for a minimum of three shifts and check in with them frequently to promote success.
  • Complete medication training and demonstrate competency in conducting medication audits on staff annually and as needed.
  • Team Lead will complete Reasonable Suspicion Education and be a part of the team to assist leadership when warranted.
  • Completes the schedule to ensure all shifts are covered and assists with filling in when and if there are holes in the schedule.
  • Will be responsible for taking 7 days or more of call per month for Height's Campuses.
  • Assists Team coordinators (Care, Food Service, Housekeeping)
  • Care Support :
  • Works with care coordinator to ensure all resident's needs are being documented and completed.

  • Provides input on care plans and ensure staff are aware of and implementing care plan goals.
  • Housekeeping Support :
  • Supports Housekeeping as needed.

  • Food Service Support
  • Ensure that food order are turned in timely.

  • Check food orders for accuracy.
  • Ensure groceries are delivered as expected.
  • Ensure that refrigerator / freezer temps are recorded daily.
  • Ensure that food storage is in compliance and well kept.
  • ESSENTIAL JOB FUNCTION : Communication / Reporting

  • Will work directly with the Assistant Director to communicate all information regarding staffing levels, staff communication and any staffing or scheduling challenges.
  • Assists coworkers with completing Incident Reports, and Medication Error Reports.
  • Meets regularly with Height's Leadership Meetings.
  • Provides oversight and meet regularly with Assistant Director to discuss Food Service, Housekeeping, Wellness / Life Enrichment.
  • Competent in completing weekly medication administration and task audits.
  • Manages tasks in Point Click Care per care plans. Will assist Assistant Director in reviewing care plans and ensure staff have signed and families have received as well as assist in coordination of and participation in care plan meetings.
  • Learns and enforces state regulations and survey process including admit requirements.
  • Coordinates and assists Assistant Director with staff meetings with feedback from nursing team and communicates outcomes to the entire team.
  • Schedules, leads, and participates in monthly resident council meetings with Assistant Health Director and communicates outcomes to Regional Communication Team.
  • ESSENTIAL JOB FUNCTION : Safety

  • ESSENTIAL JOB FUNCTION : Employee Responsibilities

  • Follows policy and procedures on TB testing.
  • Follows policy and procedures on annual influenza vaccinations.
  • Completes all mandatory training.
  • Works under the general supervision of Assistant Health Director.
  • Has contact with potential residents and their families.
  • Has daily contact with direct care staff to communicate pertinent resident admission information.
  • Performs other duties as assigned.
  • Requirements

    REQUIREMENTS :

    Minimum Requirements : Prior experience in human service or health care setting preferred. Montana Certified Nurse Aide certification preferred or must complete the SJU CNA Course and obtain certification within 120 days of class completion.

    Knowledge, skills, and abilities required :

  • Need to acquire the necessary skills and knowledge to successfully pass the required Montana State Nurse Aide Certification exam.
  • Ability to use various health care supplies and equipment, including blood pressure devices, thermometers, wheelchairs, etc.
  • Ability to understand resident charts and service plans.
  • Ability to relate to and work with the ill, elderly, disabled, emotionally upset, and at times, combative people within the facility.
  • Ability to respond to residents' needs in a timely and professional manner while recognizing the residents' rights.
  • Ability to deal with stressful situations (i.e. death & dying).
  • PHYSICAL DEMANDS : Requires moderate physical work including lifting up to 25 pounds regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and bending for prolonged periods of time.

    WORKING CONDITIONS :

  • Work involves some exposure to unusual elements such as extreme temperatures, fumes, unpleasant odors, and / or loud noises.
  • Work may involve exposure to hazards, blood borne pathogens, or physical risks, which require following basic safety precautions.
  • Normal health care environment.
  • Will receive the evening and night differential when working as a CNA.
