SUMMARY : Works under the general supervision of the Nurse Team Lead and the Director of Transitional Rehab Center. Assists individuals with a disability and / or acute / chronic illness to attain and maintain maximum function.
ESSENTIAL JOB FUNCTIONS :
Duties and Responsibilities :
- Possesses or has the ability to attain specialized knowledge and clinical skills necessary to provide care for people with physical disability and acute / chronic illness.
- Performs hands-on nursing care through applying the nursing process.
- Coordinates and delivers disease process education for patients and families.
- Collaborates with other member of the interdisciplinary rehabilitation team to facilitate achievement of patient centered goals.
- Advocates for policies, services and interventions that promote the quality of life for individuals with disabilities and participates in activities that will positively influence the community's awareness of disabilities and the role of rehabilitation
- Promotes safety by obeying safety rules of St. John's United. Identifies, reports and / or corrects obvious safety hazards and attends all mandatory safety in-services.

Employee Responsibilities :
Requirements
Minimum Requirements : Maintain Licensed Practical Nurse status in the State of Montana. Willing to obtain certification in rehabilitation nursing within the first year of employment. Must be CPR certified. Three years' experience that is directly related to the duties and responsibilities specified is preferred.
Knowledge, Skills, and Abilities :
PHYSICAL DEMANDS :
Requires moderate physical work more than 70% of the time, including lifting up to 25 pounds regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and bending for prolonged periods of time.
