Supports individuals with securing employment in the community; facilitates training and support of individuals on the job; assists with the development of individual vocational plans for each person supported; performs other job-related duties as assigned.
Essential Functions:
1. Train, supervise, and support the individual for work in the community;
2. Define individuals job criteria and implement job development activities to meet that criteria;
3. Assist in developing individual vocational plans for each person and implements services and training as detailed by same;
4. Conduct job assessments; maintain documentation of job development; job placement and other related activities;
5. Provide job coaching to individual to ensure employment success;
6. Conduct individual observations;
7. Provide transportation and/or transportation training to/for individuals during job development and/or employment;
8. Ensure that the individuals work performance is monitored and meets employer requirements and specifications for quantity and quality of work;
9. Conduct follow up support to assist individuals with the maintenance of employment;
10. Attend and participate in all relevant agency events, workshops, training and meetings;
11. Provide coverage when there is a staffing need; and
12. Performs other job-related duties as assigned.
Required Knowledge, Skills, and Abilities:
1. Ability to gain working knowledge of St. Johns Community Services policies and procedures;
2. Ability to act as a representative of St. Johns Community Services to the public;
3. Ability to effectively communicate with individuals from diverse backgrounds;
4. Ability to research, prepare, maintain and review reports and documents;
5. Ability to maintain confidentiality;
6. Ability to operate relevant computer systems, including hardware and software, and office machines;
7. Must have reliable means to transport individuals.
Physical and Environmental Conditions:
Work requires no unusual demand for physical effort.
Work environment may involve situations that require special safety precautions e.g., exposure to aggressive human behavior.

1. High School diploma or G.E.D. required; AA/BA/BS Degree in a related field preferred;
2. Two or more years related experience working with persons with disabilities required; experience in a competitive/supported employment setting preferred
3. Must meet SJCS and funding jurisdiction current requirements for criminal background/records check;
4. Valid health certificate to meet current funding jurisdiction requirements;
5. Valid state-issued drivers license with current driving record in good standing required.
