What are we looking for?
We are seeking a TBI Service Coordinator for the Syracuse Region to join our team. The Service Coordinator will provide participant-guided service coordination to individuals living in the community and enrolled in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver programs.
Essential responsibilities include the following:
- Establish and maintain regular contact, including face-to-face and in-home visits with program participants.
- Inform individuals and/or their legal guardians of their rights and responsibilities as a waiver participant, provide advocacy, and ensure that these rights are protected in service delivery.
- Complete assessments and facilitate the ongoing development of and updates to Individual Service Plans (ISPs).
- Facilitate regular Team Meetings, coordinate and monitor services among multiple providers.
- Respond to participant emergencies and address problems in service provision, support the participant in self-advocacy and problem solving.
- Provide referrals and linkages to resources, supports, and services.
- Maintain accurate and timely service records that meet regulatory requirements.
- Adherence to SJCS mission, core values, guiding principles, business standards and administrative practices.
The hiring range for this position ranges from $23 - $26 hourly and is dependent on location and experience.

- Familiarity with community-based services and resources in the designated region (currently recruiting for Syracuse and Southern Tier regions).
- Strong interpersonal skills with the ability to build rapport with participants, families, and various service providers.
- Commitment to person-centered practices and service planning led by the program participant.
- Ability to work both independently and collaboratively within a team-oriented
- Exceptional organizational, time management, and record keeping skills.
- Proficiency in Microsoft Office Suite and experience with electronic medical record/ care management databases.
Qualifications include:
- 3-6 years experience providing case management/service coordination, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors;
- Associates or Bachelor's degree in related field;
Any other acceptable combination of education and professional experience
Additional Requirements:
- Must meet SJCS and regulatory health and background requirements.
- Access to reliable transportation for reimbursable travel.
