Pay: $42.69 - $58.06 per hour depending upon experience
Job Summary:
The social worker is responsible for providing a variety of direct and related services to patients,
their families, and the professional staff. Incumbent is guided by a thorough knowledge of the
psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of
Health, Education, Welfare, and the rules & regulations of the hospital
Responsibilities:
- Completes psychosocial evaluations
- Completes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan process
- Conducts individual, group psychotherapy as prescribed by the treatment plan
- Participates in departmental meetings
- Documents according to clinical standards
- Develops after care plans for patients upon discharge including linkages to other agencies as appropriate
- Proficiency in implementing Suicide Risk Assessment
- Proficiency in implementing Telehealth Services Policy in compliance with Federal and
- NYS regulatory guidelines
Required Skills:
- Master of Social Work
- Valid NYS LCSW
- 2 years of experience in Behavioral Health Outpatient setting
