Type: Full-Time (75 hours biweekly)
Shift: Evenings
Hours: 2:30 PM -11:00 PM
Pay: $45.28 - $56.00 per hour depending upon experience Shift Differential Where Applicable
Job Description:
The Social Worker provides direct services, casework, discharge planning, and therapy to patients and families. Meets with related departments" medical staff to plan for discharges and other social work interventions.
Requirements/Skills:
- Master's Degree in Social Work from an accredited school of Social Work
- NYS LMSW or LCSW
- Previous social work experience in a hospital setting
- Experience with Mental Health patients
- Experience in Assessments, Discharge Planning, and Community Resources
- Familiar with ACS and CPS and other legal procedures
- Knowledge of CMS, DOH, TJC, and OMH regulations
- Must be computer literate, familiar with Microsoft Word and Excel
- Must be highly organized, work with the interdisciplinary team
- Must have strong verbal and written communication skills
