Description
Job Summary
The RN promotes and restores patients’ health by completing the nursing process; collaborating with physicians and multidisciplinary team members and providing physical and psychological support to patients, friends, and families.
Essential Functions
Patient Care
· Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint, vital signs, and health risk factors. Assesses disease condition using observational, interviewing, interpretation, and critical thinking skills in a developmentally appropriate manner.
·Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
· Executes prescribed treatments and medical interventions, administers prescribed medications, monitors, and documents treatment progress and patient response.
· Delegates as allowed under the Nurse Practice Act.
· Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
Care Management
· Collaborates with other clinics, community resources, providers and staff as needed in a timely and efficient manner. Ensures continuity of care.
· Educates patient and families about health status, health maintenance, and management of acute and chronic conditions.
· Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management.
Job Requirements
Minimum Education
Required: ASN
Preferred: BSN
Minimum Work Experience
Required: Must be able to perform specific clinical tasks while providing excellent clinical care. Must possess sound nursing knowledge and be able to provide/assist in patient education. Must have sound knowledge of clinic procedures. Ability to read/interpret physician notes and orders, analyze options and provide counsel to patients.
Required Licenses: Current RN license with the authority to practice in the State of Wyoming and BLS required at time of hire. ACLS desirable as well.
Functional Demands
Working Conditions
Clinic office setting. Frequent interruptions and stressful situations. Occasional overtime may be required and/or hours may be shortened as business needs. Must be on-site at the SJH campus or other facilities to fulfill the responsibilities of this role.
Physical Requirements
Requires sitting and standing associated with a normal office environment. Manual dexterity needed for using calculator and computer keyboards. Intermittent walking, standing, sitting, lifting, bending, stretching. Must be able to lift 20 pounds. Must posses sight/hearing senses to meet the requirements of this position.

Internal and External Contacts: All SJH staff, community members, patients, and their families, community and public service agencies, allied health professionals, and physicians.
