Description
Provides age and developmentally appropriate clinical care to pediatric, adult, and geriatric patient populations according to SJH Scope of Service and within Wyoming nursing license. Cross trains and floats to all outpatient specialties according to scope of practice. Floats to a different clinic day to day or week to week depending on staffing needs. Obtains adequate training to maximize versatility and proficiency in multiple clinics/departments and across the lifespan. Adheres to and St. John’s Health Corporate Compliance Program and participates in performance improvement activities.
Essential Functions
1. Patient Care
· Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint, vital signs and health risk factors. Assesses disease condition using observational, interviewing, interpretation and critical thinking skills in a developmentally appropriate manner.
2. Care Management
· Collaborates with other clinics, community resources, providers and staff as needed in a timely and efficient manner. Ensures continuity of care.
· Educates patient and families about health status, health maintenance, and management of acute and chronic conditions.
· Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management.
3. Teamwork
Collaborates with other clinics, community resources, providers and staff as needed in a timely and efficient manner.
4. Floating
Rotates to cross train in all clinics during slower periods to maintain skills and familiarity with multiple departments and workflows. Can be reassigned to another department day of depending on staffing needs of the day. Is flexible and can shift from one clinic to the next and feel efficient in each. Can work within a department solo.
Job Requirements
Minimum Education
Required: ASN
Preferred: BSN
Minimum Work Experience
Required: Must be able to perform specific clinical tasks while providing excellent clinical care. Must possess sound nursing knowledge and be able to provide/assist in patient education. Must have sound knowledge of clinic procedures. Ability to read/interpret physician notes and orders, analyze options and provide counsel to patients.
Required Licenses: Current RN License with the authority to practice in the State of Wyoming and BLS required at time of hire. Additional ACLS certification desirable.
