Job Description :
- Perform home health physical therapy in designated locations and surrounding areas.
- Deliver patient care with a 1 : 1 ratio, visiting up to 6 patients per day, with a standard of 30 productivity points.
- Participate in weekend shifts, with a requirement of 1 weekend shift per contract.
- Document patient information and treatment progress using EPIC charting system.
- Utilize personal transportation for patient visits; mileage reimbursement follows IRS Standard.
- No prior home health experience required, though Oasis experience is preferred but not mandatory.
- Work within a maximum driving radius of 50 miles when necessary, grouped to minimize drive time.
- Attend a 40-hour orientation and complete non-billable training modules pre-start and annually.
- Comply with facility's requirement to float between all locations within the state, if needed.
- Meet technical and communication needs with provided or reimbursed devices.
