St. Joseph Hospice is seeking a Part-Time Chaplain/Bereavement Coordinator to join our team!

Job duties:
- Participate in IDT meetings & will assist the IDT in evaluating and assessing patients and/or family spiritual needs
- Maintain proper records of visits as determined by the guidelines and regulations of hospice services
- Arrange for on-call availability of Chaplain services
- Work with staff, clergy and community groups to enhance their sensitivity to the spiritual concerns of patients and families
- Plans, organizes, and coordinates all bereavement activities
- Responsible for bereavement to patients family and caregivers as set forth by Medicare hospice guidelines
- Ensures documentation of all bereavement activities on a clinical note to be incorporated in the clinical record
- Clerical duties as maintaining organized volunteer records
- Provides a comprehensive orientation and training program
Qualifications:
- Graduate of accredited seminary or school of theology or appropriate certification in pastoral ministry
- Ability to be open, flexible and ecumenical
- Make patient visits at their place of residence
- Comfortable in working with patients and families with life-threatening illness and death
- Documented experience in care of bereaved
- Ability to work well with people, communicate efficiently
- Time management skills
- Must have a current LA drivers license
- Hospice or healthcare setting experience preferred but not required
