St. Joseph Hospice - Alexandria, LA is seeking Full-Time Licensed Practical Nurses (LPN) to join our patient care team.

Essential duties include:
- Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
- Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
- Document all services provided and submit on appropriate forms.
- Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
- Report changes in patient condition and needs to RN
- Great time management skills
Qualifications:
- Current Licensed Practical Nurse (LPN) in the State of Louisiana
- Minimum of 3 years as a Licensed Practical Nurse (LPN) of varied work experience, two (2) years in acute care setting and one (1) year in home care or hospice preferred.
- Have dependable transportation, drivers license and car insurance
- Previous Hospice or Home Health experience required
