Location: Inpatient Facility - Saranac Lake, NY
Position: Full-time Aftercare Counselor
Shift/schedule: Sunday-Thursday, 8:00am-4:30pm
Pay Range: $45,000.00/year to $100,000.00/year
Education and Training: High school graduate or equivalency, AAS preferred, CPR certification
Experience and Work Knowledge: Telephone skills, typing skills, standard business practices, able to work independently, computer skills and knowledge of chemical dependency helpful. CASAC preferred.
Physical Demands: No extraordinary physical demands required.
Duties and Responsibilities:
- Develop and maintain therapeutic relationship with clients.
- Actively participate in clinical supervision.
- Function as a supportive multi-disciplinary team member
- Complete thorough admission procedure for clients.
- Assists with program service delivery in absence of Primary Counselors to include admissions, assessments, treatment plan coordinator and group/individual coverage.
- Coordinating Resident transportation for discharge.
- Assist Residents in completing Aftercare referral applications.
- Conduct aftercare sessions.
- Perform discharge meetings.
- Compiling and maintaining of referral files.
- Collaborating with the Utilization Review Team for coordination of resident funding in preparation for discharge.
- Responsible for facilitation if didactic lectures as assigned.
- Facilitate phone screenings.
- Follow up with referrals to establish placements for aftercare.
- Responsible for additional duties as may be assigned by the Supervisor.
- Display professional image, appearance, and behavior reflective of SJRC Mission, policies and procedures.
- Provide weekend and holiday coverage.
Salary : $45,000 - $100,000
