What are the responsibilities and job description for the Behavior Support Professional, Part Time, 1st Shift position at St. Joseph's Center?
Essential Duties and Responsibilities:Demonstrates knowledge of and implements the assigned individual’s individual support plan (ISP).
Provides programming developed by the Behavioral Specialist.
Completes all medical appointments as scheduled, assures required documentation is accurate and complete.
Implements activities and strategies identified in the individual’s behavior support plan.
Completes all documentation and paperwork as required (client and household).
Participates in the individual’s annual ISP meeting and other meetings as required.
Responsible for the transportation needs of the assigned individual, including knowledge of vehicle policies, van logs, and adheres to transportation schedules.
Provides community based activities for assigned Individual, according to his/her community life plan.
Must attend twenty-four (24) hours of in-service training per year, inclusive of the fire safety training.
Complies with all requirements for administering medications.
Complies with all requirements for documenting on medication log.
Demonstrates knowledge of 6400 Regulations, CLA and Joseph’s policies and procedures.
Consistently shares appropriate client-center and pertinent household information to change of shift.
Complies with all tasks associated with the position and adheres to the timeframe.
Maintains a cooperative relationship with co-workers and supervisors, and creates an atmosphere of mutual respect and trust within the home and the department.
Is flexible to changes in duties, priorities, methods and environments.
Comes to work as scheduled, and is punctual.
Performs other related tasks as assigned and remains flexible to the diverse needs of the facility.
