Essential Duties and Responsibilities:
- Responsible for the medical monitoring for individuals with intellectual disabilities
- Ensure individual medical appointments are made and follow up completed
- Monitor routine labs and follow up with physicians as needed
- Responsible for the annual medical assessment and participate in team meetings
- Communicate medical issues to family members and Direct Support Staff
- Provides staff training on medical topics and techniques
Education and Experience:

- PA Practical Nursing License
- 3- 5 years experience in the ID population
- Healthcare monitoring experience
- Preferred: Certified Investigator, CPR Trainer, Medication Administration Trainer and Peer Teaching Experience.
- Valid PA Driver’s License
