Program Coordinator Essential Duties and Responsibilities:
- Ensures a safe environment for program participants and staff.
- Responsible for in-program supervision of Program Assistants, coordinating specialized training of program assistants, specific to the individual’s health and safety needs, working under the direction of the Director of Adult Day Services.
- Responsible to ensure that all Department of Public Welfare 2380 regulations are followed as well as all St. Joseph’s Center and Adult Day Program policies and procedures are followed.
- Coordinates the development and implementation of a program plan for each participant.
- Ensures that individual assessments are completed, as per 2380 regulations, and disseminated at least 30 days prior to the person’s ISP meeting.
- Must be present at the initial, annual update and revision ISP meetings for program participants.
- Coordinates the development of an appropriate program plan with an annual, quarterly, and monthly review of it.
- Ensures that ISP discrepancies are reported to the Individual’s Support Coordinator on a timely basis.
- Ensures a daily schedule of appropriate activities, which allows implementation of the program plan and program activities.
- Ensures that data is collected as per the program plan.
- Assists in meeting the personal, developmental and therapeutic needs of individual participants (changing, feeding, positioning, etc.) and ensures that these needs are met promptly, working closely with program assistants.
- Maintain all program related records based on regulations.
- Works closely with involved professionals and other team members or outside agencies that work with assigned participants.
- Assists in maintaining program materials and equipment, of sufficient type and number and will notify the Director of needs in that area.
- Participates in in-service trainings, schedule trainings when needed, and ensure Program Assistants receive trainings to include at least 24 hours of training per year as per regulatory requirements.
