Job Overview
Responsible to partner with the cardiologists, heart failure team members and clinical staff of multiple departments, both inpatient and outpatient to coordinate the care of heart failure patients. Responsible for systematically and continuously collecting and assessing data in collaboration with the multidisciplinary team to provide and improve quality patient outcomes within the Heart Failure program.
Qualifications :
- Three to five years of experience.
- Licensure required as a Registered Nurse by the State of New Jersey. Certification required as an Advanced Practice Nurse by the New Jersey Board of Nursing. Successful completion of department specific orientation and department specific certifications of Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) within one year of hire.
- Master's degree - Advanced Practice Nurse
- Work required knowledge of theories, principles, and concepts normally acquired through completion of a Master's degree in Nursing or closely related and three to five years of previous work related experience. Licensure required as an Advanced Practice Registered Nurse by the State of New Jersey. Certification required as an Advanced Practice Nurse by the New Jersey Board of Nursing. Successful completion of department specific orientation and department specific certifications of Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) within one year of hire. Effective January 2019, St. Josephs Health requires certifications for Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) to be granted through the American Heart Association.
