Role Description
This is a full-time on-site role for a Case Manager III and IV at St. Joseph's Behavioral Health located in Stockton, CA. The Case Manager will be responsible for managing client cases, providing assessments, creating and implementing care plans, conducting regular follow-ups, and coordinating services with healthcare providers. Additionally, the Case Manager will provide crisis intervention, advocacy, and support to clients and their families. The role involves collaborating with a multidisciplinary team to ensure the best possible outcomes for clients.
Qualifications
- Experience in case management, assessments, and care planning
- Crisis intervention, advocacy, and support skills
- Excellent communication and collaboration skills
- Familiarity with healthcare systems and community resources
- Ability to work independently and as part of a multidisciplinary team
- Relevant certifications or licensure (e.g., ASW, AMFT LCSW, LMFT) is a plus
- Must be registered with BBS or in process of obtaining
- Master’s degree in Social Work, Psychology, or related field
