MDS COORDINATOR JOB DESCRIPTION
FUNCTION:
The RN MDS Coordinator coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations.
QUALIFICATIONS:
1. Associate or bachelor’s degree in nursing from an accredited college or university
2. Currently licensed/registered in state of employment.
3. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
4. Two (2) years' nursing experience
5. CPR certification must remain current throughout employment.
6. Advanced knowledge in MDS process
7. Read, write, speak, and understand the English language.
8. Excellent communication and interpersonal skills.
RESPONSIBILITIES:
1. The RN MDS Coordinator coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, and regulations.
2. Perform proficiently in all competency areas including but not limited to patient rights, and safety and sanitation.
3. Maintains professional working relationships with all associates, vendors, etc
4. Maintains confidentiality of all proprietary and/or confidential information
5. Understand and follow company policies including harassment and compliance procedures.
6. Displays integrity and professionalism by adhering to the Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training
7. Report any changes in a patient's condition identified by the MDS Assessment to the DON, Unit Manager, and or staff nurse
8. Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation.
9. Assist with review of the Interdisciplinary Comprehensive Care Plan
10. Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMS iQIES system prior to billing and notify the Business Office when assessments are not ready to bill
11. Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence.
12. Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon request.
13. Exhibit excellent customer service and a positive attitude towards patients.
14. Demonstrate dependable, regular attendance in accordance with the Employee Handbook
15. Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
16. Conduct orientation sessions on the MDS process to all employees involved in the
process.
17. Provide ongoing education for employees involved in MDS documentation.
18. Maintain documentation of all orientation/training sessions.
19. Serve as a resource person for patient care planning activities.
20. Work with accounts receivable personnel to facilitate payment.
21. Maintain currency with nursing practices and educational requirements.
22. Participate in the quality improvement process of the facility.
23. Attend and participate in department/facility meetings, as required.
24. Access continuing education opportunities appropriate to responsibilities.
25. Concentrate and use reasoning skills and good judgment.
