What are the responsibilities and job description for the Affiliate Patient Navigator position at St. Jude Children's Research Hospital?
Under the general supervision of the Director, Affiliate Nursing Program (DANP), the Affiliate Patient Navigator provides coordination and communication for affiliate patients receiving care at St. Jude’s Memphis location. Manages the process to ensure patient care is coordinated with the appropriate Affiliate clinic as the patient transitions between the Affiliate clinics and St. Jude. Facilitates effective communication on all levels through tactful and professional interaction. This role will be a central contact for the patient and is primarily responsible for helping the patient navigate the transition process between the Affiliate Clinics and St. Jude Memphis.
2. Helps patients and their families understand the role of the Affiliate Clinic staff in their care.
3. Provides emotional support and related information.
4. Introduces the patient and their family remotely to affiliate staff during patient discharge process.
5. Maintains patient confidentiality.
6. Prepares monthly reports in timely manner as director by DANP.
7. Maintains personal knowledge of available resources, both in-house and outside.
8. Additionally, this role provides general administrative support for the Director of the Affiliate Nursing Program.
9 Maintains regular and predictable attendance.
10. Performs other related duties as assigned to meet the goals of the department and institution.
Associate's degree in Human Services or equivalent relevant education and experience required
Three (3) years of experience in customer service in hospital or clinic environment to include1 years of appointment scheduling.Electronic Medical Records experience preferred
Ability to multi-task between scheduling responsibilities- Ability to provide personalized customer service
- Experience providing patient instruction including direct patient interaction preferred.
- Basic working knowledge of clinical trials protocols preferred.
- Familiarity with working in a fast paced, fluid, start-up environment
- Ability to quickly gain patient trust and confidence
- Demonstrated ability with PC navigation, data entry, and phone system use
- Medical release of information experience preferred
- Must demonstrate effective recognition of cultural sensitivities
- Knowledge of medical office procedures, medical terminology and medical records.
- Knowledge of grammar, spelling, punctuation, and basic arithmetic.
- Need ability to speak clearly and concisely and to also establish and maintain effective working relationships with patients, peers and staff.
