•      Bilingual (English/Spanish)

•      Committed to providing high-quality care to underserved patients in High Desert.

•      Willingness to work as part of a collaborative team that includes, social worker, physicians, nurse practitioners, and psychiatrist.

•      St. Jude Neighborhood Health Centers utilizes EPIC for our Electronic Medical Records (EMR) system.

•      Ambulatory (outpatient) 

•      Daytime work schedule - Monday through Friday 8AM - 5PM, potential option to work remotely one day per week.

Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health. Most of our patients live below 200% of the Federal Poverty level, and are best served in Spanish. Through our origins with St. Joseph Health, we have provided care to low-income families since the 1980s, maintaining a commitment to serving our most vulnerable neighbors with high-quality, nationally recognized care. Essential Functions (What You Do)

· Conduct screening and assessments for addiction treatment;

· Provide effective follow-up services and community referrals as appropriate;

· Utilizing knowledge of a variety of therapeutic models and theories to understand and plan intervention strategies for a variety of patients;

· Working collaboratively with the patient and appropriate others to develop a treatment plan that addresses priority needs of the patient, desired treatment outcomes, an agreed upon plan of action, and reassessments;

· Build effective relationships with patients and fellow caregivers.

· Initiate and complete all state-mandated documentation within specified time frames on child and elder abuse and domestic violence.

· Maintain a safe environment for patients and colleagues, by ensuring compliance with policies, best practices, and regulatory requirements. 

· Make referrals for placement in various modalities of substance abuse and other addiction treatment, including coordinating community services.

· Maintaining patient records related to screening, intake, treatment, and continuing care that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.

· Ensure timely documentation of all patient contact in clear, concise and accurate notes with the patient’s electronic health record.

· Coordinating and cooperating with local agencies and organizations necessary to expedite treatment for each patient.

· Maintains a caseload; performs a variety of direct clinical, diagnostic, treatment, and outreach services.


Pay Range: $26.06 - $34.97 per hour

Requirements

Position Requirements

Education:
High School Graduate or GED.


License/ Certifications:
California State Certification as Addictions Specialist from a State approved certifying body (CAADE, CCAPP, CADTP)

Provider level basic life support (BLS) from AHA approved program.


Experience:
One-year recent experience, preferrable in a medical setting. 


Knowledge/Skills/Abilities: 

Office administrative practices and procedures, including HER/EMR management and the operation of standard office equipment 

Familiar with the culture and customs of the clients with whom they will interact. 

Communication skills consistent with the facilitation of substance use and harm reduction topics to individuals, small and large groups of community residents. 

Demonstrates the ability to read, write and speak English and Spanish without an interpreter. 

Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population. 

Effective written and verbal communication across all levels of the organization and with vendors.  

Highly organized with strong time management skills, being able to perform simultaneous tasks and prioritize tasks effectively and follow through in a timely manner.

Proficiency with computer software applications (e.g., Microsoft Word, Excel, Outlook, PowerPoint; Internet)


Preferred Requirements


License/ Certifications:
Post-secondary degree preferred but not required.


Experience:
Experience in FQHC or Community Health Setting 

Experience in Medical Behavioral Health Model of Practice 


Knowledge/Skills/Abilities:
Motivational Interviewing and Stages of Change models
