Requirements
The Community Health Worker is responsible for helping neighbors and health center patients navigate and access community and other services, health center services, and adopting healthy behaviors. Community Health Workers promote health education resources, connect individuals to health insurance coverage and health care services, and assess the need for and connect individuals and families to community resources. In addition, the incumbent may travel via personal vehicle throughout the community and between other Clinical facilities and departments. Ability to interact in community settings, such as health fairs, with up to 40% travel.
Shows the ability to assess and interpret age-appropriate data about the patient’s status to identify age-specific needs and provide the care needed. Demonstrate age-appropriate communication skills for the patient population served. Demonstrate knowledge of age-specific community resources. Demonstrate the skills necessary to provide care and information for vulnerable patients including those with limited health literacy.

Education:
- High School Diploma or GED
License/ Certifications:
- CPR/AED / BLS Certification
- Valid and unrestricted California Driver’s License with clean driving record.
Experience:
- 1 to 2 years of relevant work experience.
Knowledge/Skills/Abilities:
- Strong organization and follow-up skills. Ability to multi-task and prioritize.
- Knowledge of computer software programs and basic office equipment. Demonstrates the ability to read, write and speak English and Spanish without an interpreter.
- Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.
- Effective written and verbal communication across all levels of the organization and with vendors.
- Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively and follow through quickly.
- Proficiency with EMR systems.
- Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents.
- Reliable vehicular transportation to travel to community events.
- Hourly salary range $22.00 - $24.98
